When we begin working within a new senior care community, we always start by rolling out a range of assessments—including our industry-leading Age in Place Assessment, which gives us—and your team—a very clear picture of resident health in your building. This Age in Place Assessment is a score that looks at six key components to accurately measure your residents’ overall wellness and generate a prediction of the likelihood of them remaining in your community.

We’re Better at Partnerships & Consistency of Care

We partner with homecare companies who deliver Medicare Part A. Because of this partnership, we can handle the full continuum of patient care. Our other partnerships include physicians and senior care communities where we have established Therapy Rooms. Building these cross-continuum affiliations with upstream and downstream providers not only fortifies community census but delivers more consistency of care for patients.
